About the position

The RN Home Health Visiting Nurse at HarmonyCares is responsible for providing high-quality, coordinated healthcare services to patients in their homes. This role involves assessing patient needs, developing care plans, and supervising home health aide staff, ensuring that patients receive the necessary skilled nursing services. The position emphasizes communication with other healthcare professionals and requires a commitment to delivering personalized care to patients who may have difficulty accessing traditional healthcare services.

Responsibilities

  • Admit patients eligible for home care services within 24-48 hours under physician's order.
  • Assess and evaluate patient needs/problems, identifying mutually agreed upon goals with patients.
  • Report patient status and needs for other disciplines to the agency Clinical Supervisor and referring physician.
  • Develop patient care plans that specifically address identified patient problems and goals.
  • Update care plans on an ongoing basis and revise as necessary during recertification.
  • Complete admission paperwork and patient care plans submitted to Clinical Supervisor per agency policy.
  • Provide intermittent skilled nursing services including assessment, evaluation, procedures, teaching, and training activities as outlined in the patient Plan of Care.
  • Report significant findings to the patient's physician and Clinical Supervisor as they occur.
  • Submit completed skilled nursing visit notes and communication notes per agency policy.
  • Perform OASIS time point assessments per Medicare criteria and submit recertification paperwork per agency policy.
  • Maintain open lines of communication with all members of the continuum of care team.
  • Supervise Home Health Aide and document per Medicare criteria and agency policy.
  • Act as a preceptor in the orientation of new nursing staff as requested.
  • Attend staff meetings, team conferences, and educational in-services as required.
  • Participate in the discharge planning process per Medicare criteria and agency policy.
  • Follow agency policies and procedures and perform other duties as assigned.

Requirements

  • Current unencumbered State professional Nurse License.
  • Valid driver's license and good driving record.
  • Ability to make sound professional clinical judgments.
  • Ability to assess and document patient needs and formulate individualized patient care plans.
  • Proficient clinical skills.
  • Excellent verbal and written communication skills.
  • Proficiency in personal computer use, including email, clinical software, word processing, spreadsheet, and presentation software.
  • Active CPR Certification.

Nice-to-haves

  • One year of experience as a home care professional nurse and competence in performing home care comprehensive assessments.
