About the position

The RN - Home Health position is responsible for providing comprehensive nursing care to patients in their homes. This role involves assessing patient needs, developing individualized care plans, coordinating with interdisciplinary teams, and ensuring compliance with healthcare regulations. The RN will also educate patients and their families about treatment plans and manage medications and treatments effectively.

Responsibilities

• Perform accurate and complete assessments of patients, including initial admission assessments and ongoing evaluations.
• Formulate and modify care plans based on patient needs and outcomes, including teaching plans for patients and families.
• Make comprehensive nursing decisions based on patient evaluations and participate in care conferences.
• Prepare and administer medications, start intravenous lines, and maintain PICC/Central parenteral lines as needed.
• Assess the effectiveness of pain medications and revise care plans accordingly.
• Ensure documentation is complete, accurate, and timely, and communicate necessary information for evaluation.
• Comply with safety procedures to ensure a safe environment for patients and staff.
• Participate in performance improvement activities and other assigned duties.

Requirements

• Current Professional Licensure Required
• Current IL RN License Required
• BLS Certification Required
• Valid Driver's License and proof of personal auto insurance Required
• Minimum 1 year of Acute Care Nursing experience Preferred
• Minimum 1 year of Home Health Nursing experience Preferred
• Strong computer skills with the ability to navigate EMR programs
• Excellent communication and customer service skills
• Ability to work independently and prioritize tasks effectively
• Knowledge of state, federal, accreditation, and regulatory agency laws governing home healthcare
