About the position

We are seeking a compassionate and skilled Registered Nurse (RN) to join our home health team. In this part-time role, you will deliver essential medical care to patients in their homes, focusing on high-quality, compassionate care and meticulous documentation in the Electronic Medical Records (EMR) system. Your contributions will significantly impact our patients' well-being and recovery.

Responsibilities

• Perform thorough patient assessments to create and execute individualized care plans.
• Administer medications, wound care, and specialized treatments as prescribed.
• Monitor patient conditions, offer education, and support patients and families in managing illnesses and recovery.
• Coordinate care with the interdisciplinary team, ensuring holistic patient well-being, and meticulously document patient progress and care provided in the EMR system.
• Provide emotional support and empathy during challenging periods of illness and recovery.
• Adjust care plans as needed, applying critical thinking and problem-solving abilities in varied home settings.

Requirements

• Registered Nurse (RN) license in good standing and completion of a nursing program from an accredited institution.
• Prior experience in home health or a related field is preferred.
• Demonstrated empathy, sensitivity, and a patient-centered approach to care.
• Excellent verbal and written communication for effective patient and family interaction.
• Ability to work independently and adapt to different home environments while ensuring patient safety.
• High ethical standards, maintaining patient confidentiality, and respecting their autonomy.
• Proficiency in utilizing Electronic Medical Records (EMR) and other healthcare software.
• Some roles may require a valid driver's license and reliable transportation for home visits.

Nice-to-haves

• Experience with wound vacs, PICC lines, ostomy care, and catheter care.
