About the position

The RN Hospice Case Manager is responsible for providing after-hours support to patients and their families in hospice care. This role involves planning and directing services to meet individual needs within the community, particularly during evenings and weekends. The position requires a compassionate approach to end-of-life care, ensuring that patients receive the necessary support and resources in their homes.

Responsibilities

• Provide after-hours nursing support to hospice patients and their families.
• Plan and direct services to meet the needs of patients in their homes.
• Coordinate with community resources to enhance patient care.
• Ensure compliance with Medicare Hospice Certification and CHAP guidelines.
• Maintain effective communication with patients, families, and interdisciplinary teams.

Requirements

• Bachelor's or Associate's degree in nursing.
• Active CPR certification.
• 2-5 years of experience in healthcare management, hospice, or palliative care.
• 1 year of experience with Medicare Hospice Certification and CHAP guidelines.
• Proficiency in MS Office applications and ability to learn job-specific software.
• Strong organizational skills and effective communication abilities.
