About the position

The RN - Hospice Liaison plays a crucial role in facilitating smooth transitions for patients into hospice care by collaborating with healthcare professionals and educating patients about available services. This part-time position requires strong communication skills and the ability to manage complex cases while ensuring compliance with hospice eligibility and regulations.

Responsibilities

  • Collaborates and builds relationships with physicians, nurses, social workers, care managers, and community partners.
  • Attends huddles, capture meetings, discharge rounds, and outcome facilitation team meetings.
  • Communicates with healthcare professionals on complex cases to ensure care needs are met in the home setting.
  • Contacts patients upon referral to educate them on agency service lines and hospice eligibility.
  • Collects and records medical, social, and reimbursement information to initiate services for hospice and other care.
  • Educates patients and providers about financial coverage and responsibilities for ordered services.
  • May follow hospice patients readmitted to evaluate the need for continued services.
  • Demonstrates knowledge and skills necessary to provide age-appropriate care.
  • May participate in precepting new hire training and orientation.
  • Responsible for the admission to hospice services, including obtaining history/physical and implementing care plans.
  • Collaborates with business development teams to achieve admission growth.

Requirements

  • Valid RN License issued by the state of practice.
  • Graduate from an accredited school of nursing; BSN preferred.
  • Typically requires 2 years of clinical nursing experience with discharge planning and/or care management.
  • Hospice experience preferred.
  • Strong verbal and written communication skills.
  • Excellent organizational and critical thinking skills.
  • Proficient knowledge of hospice regulations and reimbursement structures (Medicare, Medicaid, Commercial Payers).
  • Strong technical ability and critical assessment skills.
  • Familiarity with Epic preferred.

Nice-to-haves

  • Experience in hospice care management.
  • Ability to train patients/families in the use of medical equipment.
