About the position

The RN - Hospice position at Mercy Hospital Oklahoma City involves providing comprehensive nursing care to patients in their homes. The role includes case management, patient assessments, and skilled nursing tasks, ensuring safety and appropriate care for patients with varying levels of acuity. The nurse will also be responsible for documentation using the Epic system and will work a full-time schedule from Monday to Friday.

Responsibilities

• Provide day-to-day patient care in a home setting.
• Manage cases and total care for multiple patients with different acuity levels.
• Conduct admissions to home care services and post-hospitalization assessments.
• Perform recertifications and discharges as needed.
• Educate patients and families on care procedures and available public programs.
• Administer IV therapy and perform dressing changes according to policy and procedure.
• Document patient care and interactions using the Epic computer system.

Requirements

• Graduation from an accredited nursing school.
• At least one year of nursing experience (LPN or RN).
• Active RN License.
• Valid driver's license and vehicle required.
• Basic Life Support (BLS) certification.
• CPR certification.
