About the position

The RN II (RN) Heart Transplant position at Cincinnati Children's Hospital focuses on providing comprehensive care for pediatric heart transplant patients. The role involves managing the post-transplant and pre-transplant processes, coordinating care with multidisciplinary teams, and ensuring compliance with medical regimens. The RN will also facilitate communication among patients, families, and healthcare providers to optimize patient outcomes and support the emotional and physical rehabilitation of transplant recipients.

Responsibilities

  • Demonstrates understanding of post-transplant/donation management including medications, complications, interventions, and discharge planning.
  • Coordinates the care of the transplant/living donor recipient in the outpatient setting to achieve optimal physical, social, and emotional rehabilitation.
  • Identifies complications and considers interventions with the transplant team.
  • Coordinates/ensures discharge planning to provide for continuity of care.
  • Assesses recipient's compliance with medication and medical regimen and collaborates with the transplant team to maintain compliance.
  • Collects and reviews data pertinent to recipient/donor health with the transplant physician.
  • Communicates adjustments in patient plan of care and drug transplant therapy as ordered by medical providers.
  • Arranges for consults, diagnostic testing, and hospitalization when indicated.
  • Coordinates emergent evaluation of transplant recipient when indicated.
  • Assists in reporting required data to regulatory bodies.
  • Participates in transplant/donation process according to specific standards during the perioperative period.
  • Shares pertinent information about the intended recipient with the transplant team.
  • Facilitates admission or transfer of patient to the transplant facility.
  • Notifies transplant team of pending medical and/or surgical procedures.
  • Removes transplant recipient/living donor from the list according to regulatory guidelines.
  • Provides outcome specifics to regulatory bodies for donor family follow-up.
  • Monitors and coordinates care of the candidate awaiting transplantation/living donation during the pre-transplant period.
  • Identifies and responds to the psychosocial and economic needs of the transplant/living donor candidate.
  • Acts as a resource for internal and external healthcare providers.
  • Facilitates/coordinates evaluation and listing of potential transplant/living donor candidates.
  • Interprets serological results and their implications for transplant.
  • Communicates effectively with patients, families, and multidisciplinary team members regarding patient clinical findings and plan of care.

Requirements

  • Bachelor's degree in Nursing or related field.
  • Current RN License.
  • 5+ years of work experience in a related job discipline.
  • ACEN or CCNE accredited BSN or MSN or Associate/Diploma RNs with 2 years experience.

Nice-to-haves

  • Master of Nursing (MSN) degree.
  • Experience in discharge planning and care plans.
  • Leadership experience.
