About the position

The RN Infusion Liaison provides direct nursing services focused on infusion therapy education for patients transitioning from hospitals to home or clinic settings. This role emphasizes patient-centered, whole-person care and requires collaboration with patients, caregivers, and healthcare providers to ensure ongoing care and education about infusion therapies.

Responsibilities

  • Provide direct nursing services related to infusion therapy for patients in various settings including hospitals, homes, and clinics.
  • Educate patients discharging from area hospitals about infusion therapies.
  • Collaborate and coordinate ongoing care with various providers, patients, caregivers, physicians, and payors.
  • Deliver infusion nursing care in home or clinic settings as needed.

Requirements

  • Oregon Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association certification within 30 days of hire.
  • Must be licensed in states wherever care is delivered upon hire.

Nice-to-haves

  • 1 year of experience with acute hospital care, infusion, or home health.
