Responsibilities

• Coordinate care for patients and families under the direction of the primary physician.
• Conduct assessments of patient and family needs.
• Develop the Interdisciplinary Group (IDG) Care Plan.
• Ensure implementation and coordination of interventions and services described in the care plan.
• Prioritize the comfort and care of patients and families.
• Provide personalized, sincere, and engaging service to patients and families.
• Address all concerns promptly and effectively.

Requirements

• RN degree from an accredited college or university.
• Registered Nurse currently licensed in the State of Florida.
• Five years of nursing experience.
• High degree of initiative and judgment to plan, coordinate, implement, and evaluate staff performance.
• Knowledge of adult learning principles.
• Experience with EMR software is a plus.
• Demonstrated proficiency in computer skills.
• Experience with oral and written communication, including public speaking and teaching.

• Bilingual (English/Spanish) preferred.
