The RN Long Term Service Program Coordinator is a part-time position based in Portsmouth, NH, with a focus on providing assessments, evaluations, planning, and community support for assigned Long Term Services (LTS) program services. The primary responsibility of this role is to determine eligibility for Personal Care Attendant (PCA) services and other in-home supports, ensuring that consumers receive the necessary assistance to promote their independence and enhance their quality of life. The coordinator will teach and provide skills training to care attendants, equipping them with the knowledge and skills needed to succeed in their roles. Additionally, the coordinator will work directly with consumers, providing training on various topics that empower them to live independently. The coordinator will review consumer care plans to ensure that their care needs are being met and will make adjustments as necessary. This role also involves assisting in obtaining services that enhance the consumers' quality of life and promoting independence. The coordinator will ensure that all paperwork is completed in accordance with Granite State Independent Living (GSIL) policies and procedures, and will assist in outreach efforts to connect with the community. As a community information and referral resource for Consumer Directed care options, the coordinator will play a vital role in connecting consumers with the services they need. Travel is required for this position, as the coordinator will participate in assessments and evaluations for eligibility in GSIL's consumer-directed in-home support programs. The coordinator will consult with individuals about their healthcare concerns, provide information on available options, and work collaboratively with care managers and case managers to address consumers' care needs. This role requires a commitment to facilitating and maximizing independence for persons with disabilities or seniors with chronic healthcare needs, ensuring that they have the support and resources necessary to live independently in the community.
