About the position

Angels of Care Home Health is seeking dedicated Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide compassionate, one-on-one medical care primarily to infants, children, and adolescents in their homes. The role emphasizes patient-centered care, focusing on the medical needs and treatment plans as directed by physicians. The position offers both full-time and part-time shifts, with a commitment to enhancing the quality of care for medically fragile patients.

Responsibilities

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct ongoing patient care and assessments.
• Administer prescribed medication, treatments, and therapies.
• Coordinate care among various healthcare providers.
• Educate family members on patient clinical care to enhance positive outcomes.
• Implement preventative initiatives to protect the quality of care for patients.

Requirements

• Active RN or LPN/LVN license (New Grads Welcome, training provided!)
• Ability to provide care in a client home setting.
• Strong interpersonal skills to make a positive and lasting impression.
