About the position

Join Love and Loyalty Home Care LLC as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide high-quality medical care to participants, ensuring their health and well-being through dedicated support and collaboration with healthcare teams.

Responsibilities

• Administer medications and closely monitor participants' health conditions to ensure optimal care.
• Provide first aid and assist with the management of chronic conditions, working collaboratively with other healthcare providers.
• Maintain accurate documentation of care provided and proactively report any changes in health status.
• Educate participants and families about health management and wellness practices.
• Collaborate with multidisciplinary teams to develop comprehensive care plans tailored to individual needs.

Requirements

• Must be a registered nurse (RN) or licensed practical nurse (LPN) with a valid state license.
• Prior experience in elderly care is preferred, with a commitment to patient-centered care.
• Strong assessment and clinical skills.
• Ability to communicate effectively with participants, families, and staff.
• Proficient in electronic health record (EHR) systems and documentation practices.
