About the position

Nuvance Health is seeking dedicated individuals to join our exceptional home healthcare team as Registered Nurses (RN) for per diem positions. Our network includes a variety of hospital and outpatient locations across Connecticut and New York, providing a comprehensive range of healthcare services. As part of the Health Quest Home Care/Western Connecticut Home Care team, you will play a crucial role in delivering high-quality care to patients in the comfort of their own homes. This position allows you to work independently while being part of a coordinated home health team, ensuring that patients receive the best possible care tailored to their individual needs. In this role, you will be responsible for performing ongoing assessments of patients, focusing on their physiological, psychological, and cognitive status. You will formulate a goal-directed plan of care based on nursing diagnoses and patient outcomes, implementing care in a knowledgeable and consistent manner. Establishing priorities based on essential patient needs and available resources is key, as is evaluating the effectiveness of the care provided and the patient's progression towards their health goals. You will also identify the learning needs of patients and their significant others, coordinating interdisciplinary functions to ensure comprehensive care. Responsibilities

  • Performs assessments in an ongoing and systematic manner, focusing on physiologic, psychological & cognitive status.
  • Formulates a goal directed plan of care which is prioritized and based on determined nursing diagnosis & patient outcomes.
  • Implements care in a knowledgeable, skillful, consistent and continuous manner.
  • Establishes priorities of patient care based on essential patient needs and available resources.
  • Evaluates effectiveness of care given and patient progression toward outcomes.
  • Identifies patient's/significant other's learning needs and implements appropriate measures to meet them.
  • Coordinates interdisciplinary functions.
  • Fulfills all compliance responsibilities related to the position.
  • Performs other duties as assigned.

Requirements

  • Registered Nurse credentials are essential.
  • Associate's Level Degree in Nursing is required.
  • Minimum experience of zero to six months is required.

Nice-to-haves

  • Nursing certification is desired.
  • Bachelor of Science in Nursing (BSN) is preferred.
