About The Position
The RN MDS Care Plan Coordinator at Apple Rehab Avon is responsible for leading the MDS department, coordinating discharge planning, and assessing short-term residents. This role focuses on maximizing reimbursement and ensuring Medicare compliance while developing and implementing comprehensive care plans tailored to each resident's needs.
