About the position
The RN MDS Care Plan Coordinator at Apple Rehab Avon is responsible for leading the MDS department, coordinating discharge planning, and assessing short-term residents. This role focuses on maximizing reimbursement and ensuring Medicare compliance while developing and implementing comprehensive care plans tailored to each resident's needs. The coordinator works closely with an interdisciplinary team to ensure that care plans are effectively communicated and followed by nursing personnel, ultimately aiming to assist residents in achieving their highest practicable level of well-being.
Responsibilities
- Lead the MDS department and coordinate discharge planning.
- Assess short-term residents and gather necessary information.
- Establish reasonable goals and provide interventions within care plans.
- Coordinate completion of comprehensive assessments by the interdisciplinary team.
- Encourage resident and responsible party participation in care plan development.
- Ensure nursing personnel are aware of and follow care plans for each resident.
- Review nurses' notes and monitor resident needs to ensure care plans are being followed.
- Plan, schedule, and conduct weekly care plan meetings according to OBRA and state requirements.
- Complete the MDS accurately to ensure the highest level of reimbursement for the facility.
- Comply with current CMS Mega Rule guidelines.
Requirements
- Current state RN license and good standing as a nurse.
- Knowledge of general, rehabilitative, and restorative nursing practices.
- Attention to detail and ability to prioritize multiple tasks.
- Ability to instruct others and follow through on tasks.
- Familiarity with laws, regulations, and guidelines governing long-term care.
