About the position

The MDS Nurse at The Laurels of Hamilton is a full-time position focused on managing the RAI process and conducting assessments and care plan coordination for residents. This role allows Registered Nurses to remain clinically involved in patient care without the demands of bedside nursing, emphasizing autonomy and organizational skills within a skilled nursing facility.

Responsibilities

  • Completes the MDS, CAA's and care plans within regulated time frames.
  • Assesses resident through physical assessment, interview and chart review.
  • Discusses resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
  • Reviews information from hospitals, consults, and outside agencies and uses such information in the completion of the assessment and care planning.
  • Coordinates, identifies, and/or initiates significant change MDS'.
  • Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator.

Requirements

  • Registered Nurse (RN) license required.
  • Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS) strongly preferred.
  • Experience as an MDS Nurse preferred, but willing to teach.
