About the position

The RN Navigator at CommonSpirit Health Mountain Region is responsible for coordinating care and services for identified patient populations across the continuum of care. This role emphasizes effective resource utilization and leadership within a multidisciplinary team to eliminate barriers to care, ensuring optimal clinical outcomes for patients and their caregivers.

Responsibilities

• Serve as the primary point of contact for identified patient populations.

Requirements

• BSN required.
• Three years of nursing experience required, with one year in specialty.
• RN license in the state of Colorado required.
• BLS through the American Heart Association required within 8 weeks of hire.

Nice-to-haves

• Previous Oncology experience preferred.
• Oncology experience in colorectal or GI highly preferred.
