About the position

The Registered Nurse (RN) position in Home Health Care involves providing and directing nursing care to patients in their homes, ensuring compliance with physician orders and applicable regulations. The role emphasizes patient independence and requires coordination with other healthcare professionals to maintain continuity of care. The RN will have the flexibility to create their own schedule while delivering compassionate care to patients.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician.
• Coordinate total plan of care with other healthcare professionals involved in patient care.
• Make initial and comprehensive nursing evaluation visits to determine the primary focus of care.
• Develop and implement the plan of care within state-specific guidelines.
• Submit accurate, complete, and timely documentation as per policy.
• Regularly evaluate and re-evaluate the patient's nursing needs.
• Perform comprehensive assessments at designated time points and develop appropriate plans of care in collaboration with physician orders.
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.

Requirements

• Current RN licensure in the state of Indiana.
• Current CPR certification required.
• Current driver's license and vehicle insurance.
• Access to a dependable vehicle.
• 1 year of nursing experience (preferred).
• 1 year of home health experience (preferred).

Nice-to-haves

• Home health experience is a plus.
• TB test required.
