Responsibilities

• Provide telephonic triage for symptoms and health concerns using established protocols.
• Utilize IT tools to improve health outcomes and facilitate referrals to the next level of care.
• Develop relationships with patients and collaborate with internal and external departments to meet patient needs.
• Participate as part of the care team and provide health information to the next level of care.
• Follow up with patients to ensure compliance with recommendations and care plans.
• Act as a subject matter expert for the Community Care Navigation program and communicate effectively with various audiences.
• Participate in quality improvement initiatives and maintain service quality standards.

Requirements

• Associate's Degree in Nursing
• 3 to 5 years of position-related experience as a Registered Nurse
• Valid Kansas Registered Nurse License
• Missouri Registered Nurse License within 30 days of hire
