About the position

The Community Care Navigator is an experienced registered nurse responsible for providing clinical triage and care navigation services to both established patients and new callers in a contact center setting. This role focuses on improving health outcomes, increasing health literacy, and facilitating referrals to appropriate care levels, while embodying the values of The Whole Care Experience.

Responsibilities

• Provide telephonic triage for symptoms and health concerns using established protocols.
• Utilize IT tools to improve health outcomes and facilitate referrals to the next level of care.
• Develop relationships and collaborate with internal and external departments to meet patient needs.
• Participate as part of the care team and provide health and plan of care information to the next setting.
• Follow up with patients to ensure compliance with recommendations and plan of care.
• Act as a subject matter expert for the Community Care Navigation program and communicate effectively with various audiences.
• Participate in quality improvement initiatives and maintain service quality and productivity standards.

Requirements

• Associate's Degree in Nursing
• 3 to 5 years of position-related experience as a Registered Nurse
• Kansas Registered Nurse License
• Missouri Registered Nurse License within 30 days of hire
