About the position

The RN Nurse Line Navigator at AdventHealth is a full-time role focused on providing telephonic triage and care navigation services to both established patients and new callers in the Shawnee Mission community. This position emphasizes improving health outcomes, increasing health literacy, and facilitating referrals to appropriate care levels, all while embodying the organization's values of holistic care.

Responsibilities

• Provide telephonic triage for symptoms and health concerns using established protocols.
• Utilize IT tools to improve health outcomes and facilitate referrals to the next level of care.
• Offer care navigation to patients and their families to enhance access and coordination of care.
• Communicate effectively with patients through phone calls, portal messages, and emails.
• Develop relationships with internal and external departments to meet patient needs.
• Participate as part of the care team and provide necessary health information to the next level of care.
• Follow up with patients to ensure compliance with recommendations and care plans.
• Act as a subject matter expert for the Community Care Navigation program.
• Participate in quality improvement initiatives and team meetings.

Requirements

• Associate's Degree in Nursing
• 3 to 5 years of position-related experience as a Registered Nurse
• Kansas Registered Nurse License
• Missouri Registered Nurse License within 30 days of hire
