About the position

The ED Case Manager RN is responsible for overseeing the patient's journey from pre-admission through discharge from the Emergency Department, ensuring a smooth transition to the community or alternate care. This role involves collaboration with various healthcare professionals to provide cost-effective and medically appropriate care, following Adena's Case Management Program to enhance quality and optimize patient outcomes.

Responsibilities

• Facilitate patient hospitalization from pre-admission through discharge.
• Collaborate with healthcare providers, nurses, and social workers to expedite care.
• Implement Adena's Case Management Program for discharge planning and resource management.
• Ensure care is provided in the most appropriate setting based on patient assessment.
• Coordinate care to improve quality, minimize costs, and ensure optimal outcomes.

Requirements

• BSN required within 3 years of hire.
• Current Ohio RN License.
• 3-5 years of acute hospital experience.

Nice-to-haves

• 3-5 years of acute hospital Case Management experience.
• Certified Case Management Certification.
• CPR certification.
