About the position
The Registered Nurse (RN) Patient Care Coordinator is responsible for managing patient care and ensuring effective communication among team members. This role involves developing and implementing nursing care plans, supervising care delivery, and evaluating patient responses to treatment. The RN plays a crucial role in coordinating care for patients and their families, ensuring high-quality nursing services are provided in a timely manner.
Responsibilities
- Manage care for patients and their families through the nursing process.
- Develop and implement nursing care plans based on patient needs.
- Supervise the delivery of care by the care delivery team.
- Evaluate patient responses to treatment and revise care plans as necessary.
- Monitor patient progress and prepare patients for discharge.
Requirements
- Associate's Degree in Nursing (Required)
- Completion of coursework from an accredited nursing school (2-4 years)
- CPR certification - AHA Healthcare Provider (Required)
- Registered Nurse (RN) license (Required)
- ACLS certification - Advanced Cardiac Life Support (Required)
- NIHSS certification - National Stroke Association NIH Stroke Scale (Required)
