About the position

The Home Health RN Patient Care Manager is responsible for overseeing and coordinating clinical services for home health patients. This role involves managing an interdisciplinary team to ensure high-quality care in accordance with physician orders and regulatory requirements. The manager is tasked with patient evaluations, benefit determinations, and the development of individualized care plans, while also ensuring effective communication among all parties involved in patient care.

Responsibilities

  • Supervise and coordinate clinical services for home health patients.
  • Manage an interdisciplinary team to ensure continuity of high-quality care.
  • Receive referrals and assign appropriate clinicians for timely patient evaluations.
  • Determine patient home health benefits and ensure ongoing insurance approvals.
  • Assess patient needs and ensure care is individualized and meets eligibility criteria.
  • Oversee the development and implementation of individualized patient care plans.
  • Document communications with physicians, clinicians, and patients regarding care updates.
  • Review assessments and plans of care daily and consult with clinicians as needed.
  • Coordinate care with all healthcare providers involved in the patient's treatment.
  • Follow up on lab tests and significant changes in patient conditions to ensure proper physician notification.
  • Facilitate case conferences and document SOC reports for effective information exchange.
  • Assist in the transfer and discharge of patients from agency services as indicated by physicians.
  • Process new orders and update visit frequencies as necessary.
  • Review clinician visit notes weekly for accuracy and completeness of documentation.
  • Enter incidents into the Risk Management Incident Reporting System as required.
  • Assist in the orientation of new agency personnel and provide leadership to clinical team members.
  • Participate in the QAPI program and ensure compliance with clinical and coding edits.
  • Direct clinicians in utilizing best practice interventions for patient care plans.
  • Participate in on-call rotation and follow up on on-call events.

Requirements

  • Current RN licensure in the state of practice.
  • Current CPR certification required.
  • Current Driver's License and vehicle insurance.
  • Access to a dependable vehicle or public transportation.
