About the position

Nuvance Health is seeking a dedicated Registered Nurse (RN) for a Per Visit position in Danbury, Connecticut. This role is part of a newly formed nonprofit health system that aims to provide communities across New York's Hudson Valley and western Connecticut with more convenient, accessible, and affordable care. As a member of the Nuvance Health team, you will be part of a network that includes several hospitals and outpatient locations, ensuring that patients receive high-quality care in a variety of settings. In this position, you will provide professional nursing care to patients by utilizing the nursing process in collaboration with other healthcare team members. Your responsibilities will include performing ongoing assessments of patients, focusing on their physiological, psychological, and cognitive status. You will formulate a goal-directed plan of care that is prioritized based on nursing diagnoses and patient outcomes. Implementing care in a knowledgeable, skillful, consistent, and continuous manner is essential to this role. You will establish priorities for patient care based on essential needs and available resources, evaluate the effectiveness of the care provided, and monitor patient progression toward desired outcomes. Additionally, you will identify the learning needs of patients and their significant others, implementing appropriate measures to address these needs. Coordination of interdisciplinary functions will also be a key aspect of your role, ensuring that all team members are aligned in their approach to patient care.

Responsibilities

  • Performs assessments in an ongoing and systematic manner, focusing on physiologic, psychological & cognitive status.

Requirements

  • Registered Nurse credentials required.
  • Associate's Level Degree in nursing is required.
  • Minimum experience of zero to six months in a nursing role.
