About the position

The RN QA Clinical Manager at CenterWell Home Health is responsible for coordinating and overseeing all direct care patient services provided by clinical personnel. This role focuses on home health clinical quality assurance and operational initiatives, ensuring compliance with regulatory standards and effective delivery of care to patients. The manager will also participate in staff training, performance evaluations, and the development of care plans, while maintaining communication with team members and stakeholders.

Responsibilities

  • Coordinate and oversee all direct care patient services provided by clinical personnel.
  • Develop, plan, implement, analyze, and organize clinical operations for the location managed.
  • Conduct and delegate patient assessments and reassessments, updating care plans as necessary.
  • Manage the assignment of caregivers and oversee the delivery of care to all patients.
  • Review patient information to determine home health or hospice needs and ensure patients meet admission criteria.
  • Instruct and guide clinicians to promote effective performance and quality home care services.
  • Assist clinicians in establishing therapeutic goals and developing patient Plans of Care (POC).
  • Monitor cases to ensure compliance with documentation requirements and regulatory agencies.
  • Coordinate communication between team members, physicians, and caregivers for appropriate care and outcome planning.
  • Work with the Branch Director and Finance Department to establish revenue and budget goals.
  • Participate in sales and marketing initiatives.
  • Supervise all clinical employees at the location, ensuring adherence to company policies and procedures.
  • Handle employee corrective actions and discipline issues in consultation with HR.
  • Participate in the interviewing, hiring, training, and development of direct care clinicians.
  • Assess staff education needs and conduct regular staff education as needed.
  • Analyze situations and identify problems using Performance Improvement principles.
  • Review clinical staff documentation for appropriateness of care and compliance with requirements.
  • Oversee QA/PI activities and collaborate with Utilization Review staff for performance review and outcomes analysis.
  • Provide direct patient care in emergencies and act as Branch Director in their absence.
  • Interpret company standards and policies to ensure compliance with regulatory authorities.
  • Maintain relationships with referral/community sources and participate in professional organizations.

Requirements

  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse.
  • Proof of current CPR certification.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Two years as a Registered Nurse with at least one year of management experience in a home care or hospice environment.
  • Home care experience is required, preferably in home health.
  • Management and people leadership experience is required.
  • OASIS experience is required.
  • OASIS certification (HCS-O, COQS, and/or COS-C) is highly desired.
  • Homecare Homebase (HCHB) experience is required.
  • CMS PDGM billing knowledge and/or experience is required.
