About the position

The RN - Registered Nurse Care Manager at Geisinger at Home plays a crucial role in the healthcare team, working closely with physicians and multidisciplinary team members to develop and implement individualized care plans for patients from admission through discharge. This position is community-based, requiring travel to patients' homes primarily within Columbia and Montour Counties, with occasional travel to Danville for meetings and coverage in surrounding areas. The RN Care Manager is responsible for continuously monitoring patient care, ensuring that patients are progressing towards their desired health outcomes through regular assessments and evaluations. In this role, the RN will assess and respond to the needs of patients and their families, coordinating the efforts of other team members to provide comprehensive care. The RN will identify and resolve barriers that may hinder effective patient care, facilitating and coordinating discharge plans to appropriate agencies. This includes serving as a resource for physicians and hospital personnel regarding available community services and ensuring adherence to quality standards through participation in quality monitoring and improvement initiatives. The RN will conduct ongoing case reviews to identify any delays or variances in care, communicating issues to the appropriate departments and physicians for resolution. The position also involves using approved medical necessity criteria to ensure that patients meet the appropriateness of their care setting, actively engaging with payer liaisons to resolve any potential issues that could lead to third-party payer denials. The RN Care Manager is accountable for complying with all organizational policies and procedures, contributing to a collaborative and supportive work environment.

Responsibilities

• Complete necessary federal, state, and local forms related to patient care.
• Ensure adherence to quality standards and participate in quality monitoring and improvement initiatives.
• Conduct ongoing case reviews for delays or variances in care and communicate issues to appropriate departments.
• Use approved medical necessity criteria to ensure appropriateness of care settings.
• Engage with payer liaisons to resolve barriers and avoid third-party payer denials.

Requirements

• Graduate from a Specialty Training Program (Required)
• Minimum of 3 years of nursing experience (Required)
• Licensed Registered Nurse in Pennsylvania (RN_State of Pennsylvania)
• Basic Life Support Certification (Default Issuing Body)

Nice-to-haves

• Prior experience in Emergency Department (ED), Intensive Care Unit (ICU), or Home Health is preferred but not required.
