About the position

As a Registered Nurse Care Manager at Geisinger at Home, you will play a pivotal role in delivering high-quality, patient-centered care to individuals in their homes. You will be responsible for assessing, planning, implementing, and evaluating the care of patients with complex health needs, ensuring that they receive the appropriate services and support to manage their conditions effectively. In this role, you will collaborate with a multidisciplinary team, including physicians, social workers, and other healthcare professionals, to develop comprehensive care plans tailored to each patient's unique needs. You will utilize your clinical expertise to educate patients and their families about their health conditions, medications, and treatment options, empowering them to take an active role in their care. Your ability to communicate effectively and build trusting relationships with patients will be essential in fostering adherence to care plans and improving health outcomes. Additionally, you will be responsible for monitoring patient progress, identifying potential complications, and making necessary adjustments to care plans as needed. You will also participate in quality improvement initiatives and contribute to the development of best practices within the Geisinger at Home program. Responsibilities

• Assess patients' health status and needs through comprehensive evaluations.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Educate patients and families about health conditions, treatment options, and self-management strategies.
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
• Coordinate care with other healthcare providers and community resources to support patient needs.
• Participate in quality improvement initiatives and contribute to best practices in home healthcare.
• Document patient care activities and maintain accurate medical records.

Requirements

• Current RN license in Pennsylvania or eligible for licensure.
• Bachelor's degree in Nursing (BSN) preferred.
• Minimum of 2 years of nursing experience, preferably in a care management or home health setting.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills to build rapport with patients and families.
• Ability to work independently and as part of a multidisciplinary team.

Nice-to-haves

• Certification in Care Management (CCM) or similar credential.
• Experience with electronic health records (EHR) systems.
• Knowledge of community resources and services available to patients.
