About the position

Grane Home Health & Hospice is seeking a passionate and dedicated Registered Nurse (RN) Case Manager to provide essential one-on-one care to patients in their homes. This role offers flexibility and autonomy, allowing the RN to grow their nursing skillset while making a meaningful impact on patients and their families during critical times.

Responsibilities

• Plan, direct, and deliver safe patient care.
• Collaborate with the Interdisciplinary Team to meet the individual needs of patients and their families.
• Provide essential one-on-one care in the patient's home.
• Support families during the hospice care process.

Requirements

• Graduate of an accredited nursing school with current licensure to practice in the state of operation.
• 1 year of nursing experience required, with 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
