About the position

The RN / Registered Nurse Case Manager - Hospice position at Grane Home Health & Hospice involves providing essential one-on-one care to patients in their homes while supporting their families. Responsibilities

• Plan, direct, and deliver safe patient care.
• Collaborate with the Interdisciplinary Team to meet patient and family needs.
• Provide one-on-one care in patients' homes.
• Support families during the hospice process.

Requirements

• Graduate of an accredited nursing school with current licensure to practice in the state of operation.
• 1 year of nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
