About the position

The Registered Nurse (RN) Case Manager position at Constellation Health Services is a full-time role focused on providing high-quality nursing care to patients in their homes. The RN will cover an assigned territory in Suffolk County, educating patients and their families on health prevention and management. The role emphasizes skilled nursing assessments, medication oversight, and collaboration with healthcare professionals to develop and implement individualized care plans.

Responsibilities

• Provide skilled nursing and head-to-toe assessments to the adult and geriatric population.
• Oversee medication administration and wound care treatment as prescribed in the physician's plan of care.
• Provide visiting home health services to an assigned caseload including admissions, re-certification, and discharges.
• Develop the Plan of Care in coordination with all healthcare professionals assigned to the case.
• Utilize the current OASIS assessment for all patient admissions, re-certification, transfers, and discharges.
• Educate patients and caregivers on disease processes, medications, and individualized treatment plans.
• Report to the Clinical Manager on a daily basis.
• Document using the provided iPad within the required time frame.

Requirements

• Licensed Registered Nurse (RN) in the state of New York.
• At least 1 year of clinical experience; home care preferred.
• Oasis experience is strongly desired.
• Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care is a plus.
• Reliable transportation and willingness to commute for in-home patient care.
