About the position
The RN Supervisor for Healthcare Services - Transition of Care at Molina Healthcare is responsible for overseeing a team of Transition of Care Case Managers who support the Medicare population. This remote position requires a Registered Nurse licensed in the state of residence, preferably California. The role involves developing care plans, assessing patient needs, and ensuring quality care throughout the patient's journey from admission to discharge.
Responsibilities
- Support a team of Transition of Care Case Managers for the Medicare population.
- Develop and implement care plans in collaboration with physicians and multidisciplinary team members.
- Assess members for care needs and create treatment plans with practitioners and support systems.
- Monitor patient care to ensure progress towards desired outcomes through continuous assessments and evaluations.
- Coordinate efforts of team members to address patient and family needs.
- Identify and resolve barriers to effective patient care.
- Coordinate medical services and community resource referrals post-discharge.
Requirements
- Active, unrestricted Healthcare Related license in California (if applicable).
- 3-5 years of experience in case management and managed care.
- Associate degree or equivalent combination of education and experience.
Nice-to-haves
- Bachelor's Degree or equivalent combination of education and experience.
- 5-7 years of experience in a similar role.
