About the position

The RN Supervisor for Healthcare Services - Transition of Care is responsible for overseeing a team of Transition of Care Case Managers who support the Medicare population. This remote position requires a Registered Nurse with leadership and case management experience to ensure quality care and effective patient outcomes. The role involves collaborating with physicians and multidisciplinary teams to develop and implement care plans, assess patient needs, and coordinate services post-discharge.

Responsibilities

• Support a team of Transition of Care Case Managers for the Medicare population.
• Develop and implement care plans in collaboration with physicians and multidisciplinary teams.
• Assess members for care needs and develop treatment plans with practitioners and support systems.
• Monitor patient care to ensure progress towards desired outcomes through continuous assessments and evaluations.
• Coordinate efforts of team members to address patient and family needs.
• Identify and resolve barriers to effective patient care.
• Coordinate medical services and community resource referrals post-discharge.
• Assist in training and orienting staff to ensure program implementation and service excellence.
• Conduct performance appraisals and monitor staff performance.

Requirements

• Active, unrestricted Healthcare Related license in California (preferred) or willingness to obtain licensure in other states.
• Associate degree or equivalent combination of education and experience.
• 3-5 years of relevant experience in healthcare.
