About the position
The RN Supervisor for Healthcare Services - Transition of Care at Molina Healthcare is responsible for overseeing a team of Transition of Care Case Managers who support the Medicare population. This remote position requires a Registered Nurse with leadership experience in case management and managed care. The role involves developing care plans, ensuring quality member care, and coordinating with multidisciplinary teams to address patient needs from admission through discharge.
Responsibilities
- Support a team of Transition of Care Case Managers for the Medicare population.
- Develop and implement care plans for assigned patients from admission through discharge.
- Assess members for care needs and develop treatment plans with practitioners and providers.
- Monitor patient care to ensure progress towards desired outcomes through continuous assessments and evaluations.
- Coordinate efforts of team members to address patient and family needs.
- Identify and resolve barriers to effective patient care.
- Coordinate medical services and community resource referrals post-discharge.
Requirements
- Active, unrestricted Healthcare Related license in California (preferred) or willingness to obtain licensure in other states.
- 3-5 years of relevant experience in case management or managed care.
- Associate degree or equivalent combination of education and experience.
Nice-to-haves
- Bachelor's Degree or equivalent combination of education and experience.
- 5-7 years of relevant experience.
