About The Position

Our Community Hospice services provides a special kind of care that focuses on allowing patients to live with dignity, where and how they want to live, with the best quality of life possible. We can provide support and guidance for months, and not just a few days or weeks. If you are an experienced RN looking for a Supervisory position and have a passion for Hospice nursing, this supervisory position leading our on call and triage teams of RNs and LPNs could be your opportunity! Work days are Monday to Friday 8am to 4:30pm. On-call rotation for evening and weekend every 6 weeks.

Requirements

• Current license to practice as a Registered Nurse in the State of New York
• Minimum of 3 years Hospice, acute care, or homecare RN experience required.

Nice To Haves

• BSN preferred but not required.
• Prior Supervisory experience strongly preferred.
• Prior Hospice experience preferred.

Responsibilities

• Lead our on call and triage teams of RNs and LPNs who are caring for Hospice patients/families in Saratoga, Washington, Montgomery, Albany, Rensselaer, Columbia, Greene and Schenectady counties.
• Coordinate closely with other RN Supervisors, the hospice interdisciplinary team, and Directors to ensure high quality end of life care to patients and families.
• Support and develop our nursing team.
