About the position

CareWell Services Southwest seeks a full-time RN to play a key role in our Supports Coordinator Team. We promote health, independence, and choice for seniors, adults with disabilities, and caregivers. As an Area Agency on Aging, we identify needs and develop plans to enable adults to maintain their independence with dignity through person-centered services. In this role, the RN will perform assessments and coordinate activities related to participants' social, emotional, and financial needs. The RN will evaluate care plans to meet individualized goals, needs, and services, ensuring that each participant receives the appropriate support tailored to their unique circumstances. The RN will conduct functional assessments of participants' social, financial, environmental, health, and emotional needs. They will facilitate and coordinate the development of a person-centered service plan of care in conjunction with the participant and other identified supports. Advocacy is a key component of this role, as the RN will arrange for care as specified in the participant's service plan. Maintaining participant electronic files and utilizing a tracking system for those receiving services is essential, as is consulting with staff to assess and manage individual case details. The RN will also be responsible for maintaining state standards of quality care and service provisions for participants. Providing information and assisting participants in connecting to community services and resources for natural support is a critical aspect of the job. Documentation is vital; the RN must document all assessments, service plans, and progress notes promptly while ensuring compliance with agency and Michigan Department of Health and Human Services (MDHHS) procedures. This position offers a flexible work schedule in a hybrid work environment, allowing for a balance between personal and professional life.

Responsibilities

Requirements

  • Must have a current Michigan Registered Nurse license.
  • Experience in care plans is preferred.
  • Bilingual is a plus.
