About the position

The Virtual Nurse (VN) at Goshen Health provides specialized remote healthcare services to patients with urgent or complex medical needs. This role is integral to the inpatient care team, utilizing digital platforms to ensure optimal care management while adhering to nursing standards and maintaining patient confidentiality. The VN is responsible for conducting virtual assessments, educating patients, documenting care processes, and facilitating care, all while participating in quality improvement initiatives and supporting individualized care plans.

Responsibilities

• Conduct virtual assessments of patients with urgent or complex medical needs.
• Educate patients about their conditions and care processes.
• Document care processes accurately in patient records.
• Facilitate care coordination and transitions across the continuum of care.
• Participate in quality improvement initiatives and professional governance.
• Utilize clinical expertise and critical thinking to support individualized care plans.
• Monitor quality metrics and ensure timely intervention for patients.
• Act as a mentor, clinician, educator, consultant, and navigator for patients.

Requirements

• Registered nurse with a valid Indiana State RN license.
• Minimum of 5 years of RN experience in an acute care hospital caring for medically complex patients.
• Strong clinical skills and knowledge of nursing standards.
• Effective communication skills and proficiency with digital health tools.
• Ability to take initiative and meet objectives.
• Strong analytical and critical thinking skills.
• Knowledge of computer use and various database software programs.

Nice-to-haves

• Experience with telehealth or virtual nursing practices.
• Certification in additional nursing specialties.
