About the position

The RN Weekend Float position at Hope Healthcare involves providing comprehensive nursing care to patients and their families in a hospice setting. The role emphasizes patient assessment, care coordination, and collaboration with an interdisciplinary team to ensure high-quality palliative care. The RN will be responsible for managing direct patient care, educating families, and supporting emotional needs during challenging times.

Responsibilities

• Assess and identify patient/family needs using the nursing process.
• Coordinate the Plan of Care with the Interdisciplinary Group (IDG).
• Provide palliative and supportive care to patients and families.
• Document patient/family contact information in the EMR and communicate with the IDG.
• Complete clinical admissions for all Company services and explain services to potential patients/families.
• Initiate skilled nursing interventions to prevent complications and maximize comfort.
• Obtain Physician orders and complete documentation per Company policy.
• Act as the Company representative at assigned facilities and facilitate referrals to all service lines.
• Provide care for GIP patients in a hospital, including daily visits and documentation.
• Participate in Quality Assessment and Performance Improvement activities.

Requirements

• Current RN license in the state of Florida.
• Minimum one year of in-patient nursing or hospice experience for external candidates.
• Previous experience working with an EMR/EHR system.
• Valid driver's license and automobile insurance.
• Ability to communicate effectively in English (verbal and written).
• Ability to manage emotional stress of working with terminally ill patients.
• Ability to physically access patient rooms and navigate various environments.

Nice-to-haves

• Experience in hospice care.
• BLS certification from the American Heart Association or Red Cross.
