About the position
The RNAC (Registered Nurse Assessment Coordinator) at WeCare at Jameson Care Center is responsible for overseeing the Resident Assessment Instrument (RAI) process, which includes the Minimum Data Set (MDS), care planning, and quality assurance activities. This role is crucial in ensuring high-quality care for residents while adhering to Medicare and Medicaid regulations. The ideal candidate will possess strong clinical assessment skills and a commitment to providing exceptional care.
Responsibilities
- Coordinate the RAI process, including completion of the MDS and care planning
- Ensure accuracy and completeness of resident assessments
- Collaborate with interdisciplinary team members to develop individualized care plans
- Monitor and evaluate the effectiveness of care plans
- Ensure compliance with Medicare and Medicaid regulations
- Provide education and training to staff on the RAI process and care planning
- Perform all other job duties as assigned by administration
Requirements
- Current RN license in the state of PA
- Certification in RAC-CT preferred
- Strong clinical assessment skills
- Knowledge of Medicare and Medicaid regulations
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
