About the position

The School Based Behavioral Health Liaison will work within the Willson County School System to provide comprehensive care management and support services to children and families. This role focuses on enhancing access to medical, social, educational, and supportive services, ensuring treatment adherence, and facilitating communication between various stakeholders including families, schools, and healthcare providers.

Responsibilities

• Provide comprehensive care management and monitor progress of person-centered care plans.
• Participate in the patient's physical health treatment plan and support scheduling for appointments.
• Identify and facilitate access to community supports and resources for patients.
• Provide high-touch in-person support to ensure treatment and medication adherence.
• Develop discharge plans and ensure timely access to follow-up care post hospitalization.
• Educate patients and families on independent living skills and health promotion.

Requirements

• Master's Degree in Behavioral Sciences (social work, counseling, or psychology).
• Experience in group, family, and individual therapy preferred.
• Excellent communication and networking skills.

• Familiarity with IEP processes and educational support systems.
