About the position
The School Based Licensed Practical Nurse (LPN) at Meridian Health Services is responsible for providing comprehensive healthcare services to students in a school setting. This role emphasizes a holistic approach to health, integrating physical, mental, and social well-being. The LPN will work closely with medical staff to manage medication regimens, conduct health screenings, and provide education to students and their caregivers.
Responsibilities
- Respond to requests from students/caregivers regarding medication effectiveness and side effects, consulting with medical staff as necessary.
- Oversee documentation in the Medication Administration Record (MAR) and make corrections as needed.
- Document school-based visits in the electronic health record system.
- Phone in prescription orders to pharmacies as directed by medical staff.
- Review lab and test results, reporting concerns to medical staff.
- Obtain medical histories and perform physical assessments, including vital signs and point-of-care testing.
- Administer injectable medications as ordered by medical staff.
- Provide educational groups and one-on-one teaching regarding medications and health issues.
- Complete required clinical documents for review by medical staff.
- Participate in treatment planning and modifications for students/clients.
- Assist in maintaining student health records and follow up on immunizations.
- Act as a liaison with students/caregivers and healthcare providers to clarify medication orders and obtain necessary referrals.
- Complete documentation for accurate billing.
Requirements
- Must be a Licensed Practical Nurse with an accredited nursing program completion and current Indiana license.
- Valid driver's license, proof of auto insurance, and reliable transportation.
- Two years of nursing experience required.
