The Pharmacy Technician for Medication Reconciliation at ProHealth Care plays a crucial role in ensuring the accuracy of medication management for patients during their hospital stay. This position involves assisting pharmacists, physicians, and nurses in the medication reconciliation process, which includes verifying and coordinating medications upon admission and discharge. The role is integral to maintaining patient safety and enhancing the quality of care provided by the healthcare team.

Responsibilities

• Assists pharmacists, physicians, and nurses with medication reconciliation processes to ensure intentional medication orders.
• Completes medication histories for new admissions.
• Promotes the hospital's retail pharmacy program through discharge prescription services.
• Collects appropriate insurance and demographic data for prescription processing.
• Participates in quality assurance programs and contributes to decision-making processes.
• Maintains updated knowledge and skills and contributes to the education of others.
• Provides age/developmentally appropriate care in accordance with guidelines.

Requirements

• High School Diploma or equivalent
• Pharmacy Technician (CPhT) Registered with the State of Wisconsin
• Valid WI driver's license and in good standing
• Basic Life Support Certification (BLS)
