About the position

The Senior Social Worker in the HUD-VASH program at the Department of Veterans Affairs is responsible for providing intensive case management to chronically homeless Veterans and those with complex health care needs. This role focuses on outreach, assessment, service coordination, and linking Veterans to permanent supportive housing, ensuring they receive the necessary support to maintain stable housing and improve their quality of life.

Responsibilities

  • Provide clinical case consultation on high-risk Veterans with colleagues in the VA and community-based programs.
  • Coordinate prioritized admissions to permanent supportive housing programs in the VA and community.
  • Review lease agreements between the local Public Housing Authority, landlords, and property management groups.
  • Provide service coordination to assist Veterans in securing and maintaining permanent housing.
  • Perform assessments with HUD-VASH participants in various settings to ensure safe living environments.
  • Advocate for emergency transfers in accordance with the Violence Against Women Act (VAWA).
  • Reassess housing and clinical stability, plan critical time interventions, and update treatment planning.
  • Collect and ensure compliance with HUD-VASH Program participant data as per documentation policies.
  • Coordinate care and link Veterans to VA health care facilities and community resources.
  • Participate in monthly VISN 12 and National HUD-VASH Program conference calls and trainings.
  • Assist in developing or modifying Standard Operating Procedures (SOPs) for community needs.
  • Participate in program planning and development, advising on trends and recommendations for policy changes.

Requirements

  • Must be a U.S. Citizen.
  • Must have a Master's degree in Social Work (MSW) from an accredited school.
  • Must be licensed or certified by a state at the advanced practice level.
  • At least two years of experience post-licensure in a specialized area of social work, with one year equivalent to GS-11 level.
  • Demonstrated advanced practice skills and judgment in social work.

Nice-to-haves

  • Experience with HUD-VASH or similar programs.
  • Knowledge of local housing codes and Public Housing Authority guidelines.
  • Familiarity with the Violence Against Women Act (VAWA) and its implications for housing.
