About the position

The Service Coordinator role at AmeriHealth Caritas is a remote, field-based position focused on assisting Medicaid participants in accessing long-term services and support (LTSS) as outlined by the Community Health Choices (CHC) agreement. The role involves conducting assessments, coordinating care, and managing participants' needs across various health and social services. The ideal candidate will have a strong background in social services or healthcare, with a passion for helping individuals improve their quality of life.

Responsibilities

  • Assist participants in obtaining long-term services and support as required by the CHC agreement.
  • Perform telephonic and face-to-face assessments to evaluate and manage participants' needs.
  • Identify and coordinate access to needed LTSS services and other covered services.
  • Lead the participant-centered planning and service delivery process and oversee its implementation.
  • Inform participants about available assessments, service alternatives, and delivery options.
  • Assist participants with the complaint, grievance, and DHS Fair Hearing process.
  • Collect necessary information to inform the development of the participant-centered service plan (PCSP).
  • Conduct annual reevaluations of level of care as needed following CHC requirements.
  • Maintain eligibility by working with participants to complete necessary activities.

Requirements

  • 3+ years of social service or related healthcare experience.
  • Experience working with individuals needing personal care services.
  • Conducting face-to-face visits focused on needs assessments and service monitoring.
  • Knowledge of the home and community-based service system.
  • Maintaining contact with healthcare professionals and providing timely feedback.
  • Valid and current driver's license.
  • Access to reliable automobile transportation with state-mandated minimum insurance.
  • Computer proficiency and working knowledge of MS Office.
  • Bachelor's Degree in social work, psychology, or related field preferred.
