The Virtual Care Service Coordinator plays a crucial role in managing and coordinating services provided by Texas Health's Virtual Care Delivery Channel. This position is designed for individuals who are proactive problem solvers with excellent organizational skills, capable of handling a diverse patient population and working collaboratively with a multidisciplinary team. The role emphasizes high-quality service delivery, effective communication, and the ability to adapt to a dynamic healthcare environment.

Responsibilities

  • Manage multiple services provided by Texas Health's Virtual Care Delivery Channel to ensure seamless care
  • Coordinate and schedule visits as needed
  • Manage complex and multidisciplinary real-time scheduling needs of a diverse patient population
  • Diligently follow-up on assigned tasks and communicate updates to the team in an efficient and reliable manner
  • Collect and organize patient data

Requirements

  • HS Diploma or Equivalent Required
  • 1 Year Experience related experience Required
  • Independent, proactive problem-solving skills
  • Excellent organizational skills with attention to detail
  • High level of service deliverability including multi-tasking, ability to prioritize work and meet deadlines and adapt to a dynamic environment
  • Strong written and verbal communication skills
  • Computer literacy, including command of Microsoft Word, PowerPoint, and Excel with ability to learn new software as needed
  • Ability to demonstrate respect and professionalism for patients' rights and individual needs, including demonstrated ability to maintain HIPAA compliance and other confidentiality
  • Ability to work independently and as a team player
  • Analytical skills and ability to resolve technical problems if needed
  • Excellent customer service skills, including the ability to manage and triage complex telephone calls
  • Comfortable working with a diverse patient population and multidisciplinary team, including nursing, physicians, and ancillary service providers such as PT, OT, skilled nursing, etc.
  • Ability to maintain a positive attitude and uphold the ACH culture
  • Proficient in handling and troubleshooting multiple devices and platforms
