About the position

The Service Coordinator at San Gabriel/Pomona Regional Center (SG/PRC) is responsible for providing service coordination to individuals with intellectual and developmental disabilities and their families. This role involves consulting with various stakeholders to arrange necessary services, developing Individual Program Plans (IPP), and ensuring compliance with documentation timelines. The position requires a strong understanding of the needs of individuals with developmental disabilities and the ability to communicate effectively with diverse populations.

Responsibilities

  • Consults with Manager, appropriate liaisons, and specialist staff to arrange services for individuals served.
  • Explains available resources and services to individuals and their families.
  • Conducts in-person meetings with individuals, families, vendors, and other agencies.
  • Develops and implements Individual Program Plans (IPP).
  • Documents interim and annual reports within mandated timelines.
  • Writes inter-disciplinary notes.
  • Initiates purchase of service (POS) through the Regional Center for individuals ineligible for other funding sources.
  • Presents cases at specialized committees as needed.
  • Manages all aspects of the case management process.
  • Performs other duties as required.

Requirements

  • Bachelor's Degree in social work, psychology, human development, sociology, public health nursing, or a related field.
  • One year of experience in developmental disabilities or a related field (Master's degree can substitute for experience).
  • Knowledge of individuals with developmental disabilities.
  • Interview, counseling, and crisis intervention techniques.
  • Ability to independently plan and schedule work.
  • Ability to assess clients' needs and formulate goals and objectives.
  • Effective verbal and written communication skills.
  • Ability to multi-task and set priorities.
  • Ability to meet strict documentation timelines.
  • Ability to interact with individuals from diverse cultural and social backgrounds.
  • Knowledge of word processing software (Microsoft Word).

Nice-to-haves

  • Bilingual in Chinese/Mandarin, Vietnamese, Korean, or Spanish.
