About the position

The Service Coordinator at Concern Housing is responsible for providing essential support and advocacy for clients, linking them with community resources, and assisting with daily living skills. This role involves counseling, documentation, and ensuring compliance with program requirements, making it a vital position for client care and community integration.

Responsibilities

• Facilitate residents' linkage with community services such as case management and vocational services.
• Develop quarterly goal plans and conduct functional assessments.
• Hold weekly meetings with clients to develop skills and review progress.
• Maintain all documentation required for client records, including progress notes and discharge plans.
• Provide individual counseling and support services.
• Assist with housing services and maintenance issues.
• Conduct daily living skills training, including medication monitoring and money management.
• Perform monthly fire safety inspections.
• Attend weekly supervision and staff meetings.
• Stay current with all agency-required compliance documents and trainings.
• Complete other duties as assigned by the Program Director or Supervisor.

Requirements

• Bachelor's Degree in Psychology, Rehabilitation, Social Work, or a related mental health discipline.
• 2 years of experience working in a similar setting or with people with psychiatric disabilities.
• Strong verbal and written communication skills.
• Ability to make sound judgments regarding client care.
