About the position

As a Case Manager II at an in-custody (jails) program, your role will involve providing comprehensive support services to individuals who are incarcerated. You will be responsible for assessing participants' needs, developing individualized service plans, and connecting them with appropriate resources to help them regain stability and move towards permanent housing. Your primary objective will be to empower clients to overcome barriers and achieve self-sufficiency.

Responsibilities

  • Conduct comprehensive assessments of individuals experiencing homelessness to identify their needs, strengths, and goals.
  • Develop individualized case plans in collaboration with clients, addressing their immediate and long-term needs, such as housing, employment, healthcare, and social support.
  • Connect clients with appropriate resources and services, such as housing programs, mental health services, substance abuse treatment, job training, and legal assistance.
  • Advocate for clients' rights and needs within the shelter and in the community, including liaising with social service agencies, government entities, and community organizations.
  • Provide immediate support and intervention during crisis situations, such as mental health crises, substance abuse relapses, domestic violence incidents, or medical emergencies.
  • Regularly monitor clients' progress, document interactions, and maintain accurate and confidential case records. Update case plans as necessary based on clients' changing needs.
  • Offer emotional support, guidance, and counseling to clients, helping them develop coping skills, problem-solving abilities, and positive relationships.
  • Facilitate workshops and group sessions on life skills, including budgeting, job readiness, communication skills, and conflict resolution.
  • Collaborate with other case managers, shelter staff, and external service providers to ensure coordinated and holistic care for clients.
  • Assist in evaluating the effectiveness of the shelter's programs and services, providing feedback and recommendations for improvement.
  • Stay updated on best practices in case management, homelessness, and related fields through ongoing training and professional development opportunities.

Requirements

  • Experience working with vulnerable populations, volunteering at homeless shelters or community organizations, or participating in internships or practicums in social services.
  • Basic knowledge and understanding of homelessness issues, social services, and community resources in San Francisco.
  • Familiarity with trauma-informed care, cultural competency, and client-centered approaches.
  • Knowledge of relevant laws, regulations, and policies related to homelessness and social services.
  • Excellent interpersonal and communication skills to establish rapport, actively listen, and effectively communicate with clients, colleagues, and external stakeholders.
  • Basic organizational and time management skills to handle caseloads, maintain documentation, and meet deadlines.
  • Empathy and compassion to provide support and assistance to individuals experiencing homelessness.
  • Problem-solving skills to identify client needs, navigate resources, and address barriers.
  • Cultural sensitivity and the ability to work with individuals from diverse backgrounds.
  • Bachelor's Degree in related field or a minimum of 4 years direct experience in case management.

  • Certified Case Manager (CCM), Certified Alcohol and Drug Counselor (CADC), or Mental Health First Aid.
