About the position

The Site of Care (SOC) Operations Manager at CVS Health is responsible for enhancing operational processes and implementing action plans to achieve organizational goals. This role focuses on transforming care delivery, fostering collaboration between clinical services and provider networks, and driving improvements in quality and patient experience in post-acute care. The manager leads a team that engages with providers, analyzes performance, and develops strategic roadmaps to ensure effective care transitions and operational efficiency.

Responsibilities

  • Collaborate with Partner Success, Operations leadership, and Clinical Teams to align operational interventions with overall strategy.
  • Ensure program execution aligns with plans, partner contract requirements, and company commitments.
  • Deliver interventions to improve Value-based care programs.
  • Establish and nurture partner relationships to support collaboration and execution of priorities and goals.
  • Manage Site of Care operations staff to ensure effective communication and training.
  • Provide program level management of day-to-day operations.
  • Review client data, action plans, operational plans, and escalations.
  • Conduct 1:1 meetings with staff to discuss data and challenges.
  • Facilitate Client Team Meetings to review actions and share data.
  • Collaborate with CVS Accountable Care partners to identify solutions for market inconsistencies.
  • Capture marketplace feedback to enhance Care Transition strategies and elevate to Senior Leadership for improvement.
  • Motivate and mentor staff to improve processes and results.
  • Contribute to a motivated work environment to achieve common goals.

Requirements

  • 7+ years of experience in health care delivery methods and tools, clinical performance improvement, clinical operations, population health, and care management.
  • 5+ years of experience in Medicare and clinical program delivery and performance improvement techniques.
  • 3+ years of demonstrated leadership and coaching skills, with a preference for prior management experience.
  • Expertise in interpreting population data and risk stratification.
  • Experience working with clinical and claims data.
  • Consulting or business development experience is a plus.
  • Experience implementing innovative service models that drive operational results in health fields.
  • Experience with Episodes of Care or Value Based Care.
  • Strong critical thinking skills to analyze medical data and reports.
  • Experience with Health Care Organizations.
  • Ability to travel up to 50%.

Nice-to-haves

  • Master's degree in a related field (MBA, MHA, MPH) or other health professional degree (RN, etc.).
  • Consulting or business development experience.
