About the position

The Skill Builder position at Meridian Health Services focuses on providing skills and development training to assist patients and families in achieving developmental milestones and overcoming psychosocial barriers. This role involves case management services and collaboration with the clinical team to implement treatment plans effectively, ensuring a holistic approach to healthcare that integrates physical, mental, and social well-being.

Responsibilities

  • Assists the Therapist in developing, implementing, and revising individual treatment plans, ensuring services align with the treatment plan and monitoring progress toward goals.
  • Provides individual and group skills training for adults and children to develop skills such as self-care, daily life management, interpersonal communication, and positive coping skills.
  • Delivers case management services as outlined in the patient's treatment plan.
  • Collaborates with the Therapist and medical staff to ensure integrated care coordination, addressing health issues and treatment compliance.
  • Completes CANS/ANSA re-assessments as required.
  • Consults with staff on specific cases to facilitate coordination of care.
  • Maintains positive relationships with staff from other agencies and acts as a liaison between patients and institutions.
  • Prepares and maintains clinical records, reports, and other administrative tasks as needed.
  • Participates in continuing education to stay knowledgeable in areas of expertise.

Requirements

  • High school diploma or equivalent with 2 years of paid behavioral healthcare experience or equivalent college coursework in human services or health-related fields.
  • Valid driver's license with a driving record that meets company policy, reliable transportation, and proof of auto insurance.
  • Commitment to providing individual attention to help clients achieve mental, emotional, and physical well-being.

Nice-to-haves

  • Associate or bachelor's degree in a relevant field preferred.
