About the position

The Skilled Inpatient Care Coordinator (SICC) plays a crucial role in optimizing patients' recovery journeys by coordinating discharge planning and facilitating informed decisions for patients and their families. This position is integral to ensuring smooth transitions of care from hospital to home, particularly for older adults, and involves collaboration with various healthcare professionals to support patients' post-acute care journeys.

Responsibilities

  • Serve as the link between patients and healthcare personnel to ensure efficient transitions of care.
  • Perform Skilled Nursing Facility (SNF) assessments on patients using clinical skills and CMS criteria.
  • Review targets for Length of Stay (LOS), outcomes, and discharge plans with providers and families.
  • Complete SNF concurrent reviews and update authorizations timely.
  • Collaborate with healthcare teams to establish optimal discharge plans.
  • Monitor patients' progress toward discharge goals and assist in resolving barriers.
  • Participate in SNF Rounds and provide updates to management.
  • Ensure appropriate referrals to Health Plan and community services are made.
  • Engage with patients and families weekly, either telephonically or on-site.
  • Attend patient/family care conferences.
  • Assess patients' continued appropriateness for SNF setting according to CMS criteria.
  • Coordinate peer-to-peer reviews with medical directors.
  • Support new contract start-ups with experienced staff.
  • Manage assigned caseload efficiently using time management skills.
  • Enter timely and accurate documentation into nH coordinate.
  • Review census daily and identify barriers to workload management.
  • Review monthly dashboards and reports with Clinical Team Manager for improvement opportunities.
  • Adhere to organizational policies and maintain confidentiality of PHI information.
  • Complete cross-training to support coverage needs across the business.
  • Stay current on federal and state regulatory policies related to care coordination.
  • Promote a positive work environment and attend meetings as requested.

Requirements

  • Active, unrestricted registered clinical license in state of hire (Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist).
  • 5+ years of clinical experience.
  • Reside within or near Surprise, AZ or Peoria, AZ.

Nice-to-haves

  • Familiarity with care management and utilization/resource management processes.
  • Proficient with Microsoft Office applications (Outlook, Excel, PowerPoint).
  • Experience working with the geriatric population.
  • Background in patient education, rehabilitation, or home health nursing.
  • Detail-oriented with strong prioritization and planning skills.
  • Exceptional verbal and written communication skills.
  • Solid problem-solving and conflict resolution skills.
  • Independent decision-making skills.
